# Missing details before a first physiotherapy visit

A first appointment is approaching and the practice has received a form, but one administrative field is empty. The owner wants the reception team to ask for the right detail without sending the person back through the whole form.
This fictional example uses a short checklist chosen by the practice. It concerns contact details, while questions about symptoms or treatment stay with the appropriate clinician.

## Fictional example

The owner's request: Check P-14's first-visit form against our administrative checklist and draft a request for what is missing.

Information provided:
- Checklist: name, telephone number and preferred contact method.
- P-14 form: name provided; telephone blank; preferred contact method is email.
- Appointment record: email is an approved contact route.

Example result: Missing from the form: telephone number. Draft: 'Hello, we have your first-visit form. Could you provide a telephone number for your appointment record? Thank you.'

The next step: The owner asks reception to check the approved contact route and the wording; the practice records the number only if it is provided.

## Compare the form with the practice's checklist

For appointment P-14, the practice's sample checklist asks for a name, telephone number and preferred contact method. The form contains the name and says email is preferred; the telephone field is blank.

The appointment record separately shows an approved email contact route. The missing field can therefore be identified without interpreting anything about the person's health.

## Ask for the missing item, not the whole form

The assistant can help prepare a short request for the telephone number. The person does not have to fill in their name and preferred contact method again.

Before using it, reception checks that the contact route is still appropriate. If a patient replies with a treatment question, the team routes that question to a clinician rather than treating the administrative draft as advice.

## Update the record after the answer

In the example, the owner chooses to have reception review the request and use the approved route. The blank remains open until the practice receives the information and records it in the usual place.

If the person does not wish to provide a number, reception follows the practice's process for resolving the missing field instead of inventing a value.

## Questions you may have

### Does the assistant decide what information a patient must provide?

No. The practice supplies its own administrative checklist and decides how to handle an unanswered field.

### What happens if a reply contains a question about treatment?

The practice routes it to the appropriate clinician. The example is limited to an administrative request.

### Is the request sent as soon as it is prepared?

No. Reception reviews the draft and uses the practice's approved contact process.
